If temperature rises faster than the expected rate during addition, stop or slow addition immediately and increase cooling before continuing — regardless of how much of the batch remains. 4. If any deviation from the batch record occurs, evaluate its chemical consequence before releasing the batch, rather than relying on visual or physical inspection alone. 5. Apply lockout/tagout procedures before entering or servicing any equipment that processed a hazardous chemical, regardless of apparent current state. 6. Verify secondary containment and spill response readiness on a scheduled basis, not only after an incident. 7. To EHS (environmental health & safety): leads with any LOTO status or containment readiness issue in the specific terms the safety program tracks. To quality/lab: leads with the specific batch deviation and what analytical test is needed to characterize its impact, not a request for a general "is this batch okay" judgment. ## Common failure modes - Adding reagents on a fixed time/volume schedule regardless of actual reactor temperature response, missing an accelerating exotherm until it's already a runaway. - Reordering a reagent addition sequence for convenience based on an assumption that the final mixture will be chemically equivalent. - Releasing a batch with a documented deviation based on visual or physical inspection alone, without an appropriate analytical check for chemical consequence. - Treating lockout/tagout as unnecessary for equipment that "looks" empty or already purged. - Having learned to stop or slow addition at the first sign of temperature rise, over-applying extreme caution even to a well-characterized, mild exotherm that's expected and within normal control range. ## Worked example A batch reactor charges Reagent A (200 kg total) into 500L of solvent over a planned 90 minutes (2.22 kg/min average rate), an exothermic addition with reactor temperature specified to stay ≤45°C, matched to the cooling jacket's demonstrated capacity at that addition rate. At t=40 minutes, 100 kg has been added (on pace) with reactor temperature at 44°C — within spec. By t=45 minutes, cumulative addition reaches 112 kg, but reactor temperature has risen to **48°C — 3°C over the 45°C limit**, not a stable elevated reading but an active excursion. **Naive read:** with 112 kg (56%) already added and only 88 kg remaining, the operator continues addition per the original 90-minute schedule, assuming the temperature will stabilize on its own since the batch is "almost done," planning only to adjust cooling in parallel. **Expert approach:** a temperature excursion appearing *after* the reaction had been running within spec is the specific signature of heat generation beginning to outpace cooling capacity — the leading indicator of thermal runaway risk, not a value to tolerate because most of the addition is complete. Addition is stopped immediately at t=45 minutes (112 kg added, 88 kg remaining), cooling is increased (jacket coolant setpoint lowered, agitation increased for heat transfer), and temperature is monitored until it peaks and begins declining — confirmed under control when it peaks at 49.5°C at t=48 minutes and falls back to 44°C by t=55 minutes. Addition resumes at a reduced rate: the remaining 88 kg is added over 55 minutes instead of the originally-remaining ~40 minutes, a rate of **1.6 kg/min** — comfortably below the 2.22 kg/min rate that triggered the excursion, staying within the cooling system's demonstrated margin. Total batch time extends from the planned 90 minutes to roughly 110 minutes (a 22% extension), but the batch completes without a thermal runaway event. **Deliverable (batch record deviation entry):** > Batch #6604, Reagent A addition. Planned: 200 kg / 90 min (2.22 kg/min), temp ≤45°C. Deviation: at t=45 min (112 kg added), temp reached 48°C (3°C over limit) — active excursion, not stable elevated reading. Action: addition STOPPED (not continued per schedule); cooling increased. Temp peaked 49.5°C at t=48 min, returned to 44°C by t=55 min.
